Our A5 and A4 breakdown recovery invoice books are made to improve the efficiency of your automotive business operations. All sheets are printed in full colour. Available in convenient 2-part books, they are ideal for on the go businesses. Also used as proof of breakdown recovery.

The form is laid out for a recovery job rather than a general invoice. The vehicle block records make, model, registration, colour, fuel and transmission type, whether you have the keys, load, passengers and any caravan or trailer, then recovery location, where it was recovered to and who received it. Times are captured separately for arrival, roadside and recovery, alongside recovery mileage and any faults or parts fitted.

A Visual Damage Record with a four-view car outline lets the driver mark existing damage with an X before the vehicle moves, and three signed declarations below it cover accidental damage during recovery, authority to gain access, and delivery of the vehicle. The charges column splits call out, roadside, parts and recovery into a subtotal and total.

Currently, we offer a free design service, using our existing templates to create a layout that meets your needs (please note, logo design is not included). But, we can incorporate your pre-existing, print-ready designs at no additional charge.

Each pad is stapled, glued and has top perforation for easy separation of sheets. Alternatively, we offer a glued-only option if you prefer. Let us know your preference!

We also provide free sequential numbering for your invoice books. Please inform us of your specific numbering requirements.

To ensure the utmost functionality, every pad comes with a sturdy 400gsm white board placed between sets, preventing any transfer to the following set.

What is on a breakdown recovery invoice?

Customer name, address and telephone, invoice number, invoice date and recovery date, then a full vehicle block covering make, model, registration, colour, fuel and transmission type, keys, load, passengers and any caravan or trailer. Below that: recovery location, where the vehicle was recovered to, who received it, arrival, roadside and recovery times, recovery mileage, faults or parts fitted, a Visual Damage Record with a car outline, and a charges column splitting call out, roadside, parts and recovery.

What is the Visual Damage Record for?

It is a four-view outline of a vehicle printed on every set, on both A5 and A4. The driver marks existing damage with an X before the vehicle is moved, and the customer signs three declarations covering accidental damage during recovery, authority to gain access to the vehicle, and delivery. It gives you a signed record of the vehicle’s condition at the roadside.

Can I change the fields on the form?

Yes, free of charge. Any line, box or declaration can be amended, added or removed, and your logo and contact details are printed at the top. If you have your own print-ready design we print that at no extra cost.

What sizes and parts are available?

A5 (148 x 210mm) or A4 (210 x 297mm), duplicate two-part NCR — white top copy, yellow second copy — 50 sets per book. Top perforation, glued and stapled as standard, or glued only if you prefer. Free sequential numbering on request.
